Typed, purchase of service agreement between HQB and the Ramsey County Welfare Department. An excerpt form the document lists the following proposed service:
Home Delivered Meals to be provided daily to 27 homebound Seniors and disabled persons. Transportation to medical and government facilities for shopping, medical appointments and other tranportation as needed. Informational and referral services, if agency is unable to directly provide services need.
[...]Outreach services will be provided to insure those people in need of services will be reached and served. Health services to expand and identify in coordination with the Model Cities Health Clinic.
