This Certifies that Matthew Carey 8lbs. 3 1/2ozs. was born to Mr. amd Mrs. Robert Jr. Montgomery in this Hospital at 4:19 p.m. CDT July 23, 1967.
In Witness Whereof the said Hospital has caused this Certificate to be signed by its duly authorized officer and the attending physician and has hereunto affixed its Offical Seal.
The birth certificate is signed by the attending physician and administrator of the hospital and is stamped with an embossed seal. A aerial photo of the hospital is printed at the upper right of the page.
The back side of the certificate has Matthew's footprint in ink as a blank form where one could fill out family history details.
